No default Avatar

Question: 

Following the instructions on page 36 of Dorien's great publication (Thanks!). My uploaded avatar displays fine, but when no Avatar is uploaded the default avatar isn't displayed. Just get: <?php echo ''; ?> where the image should be.

All suggestions appreciated!
